What You’ll Do:
- Coordinate Care: Assist in identifying patients for specialized care programs and manage their progress with personalized care plans.
- Educate & Empower: Deliver individual and group education on disease management, healthy lifestyle changes, and preventive care. Use a mix of in-person, telephonic, and virtual platforms to reach patients.
- Health Coaching: Offer one-on-one and group coaching to encourage behavior modification and promote wellness.
- Collaborate: Work alongside a multidisciplinary team, including providers and medical directors, to ensure seamless patient care and improve outcomes.
- Community Support: Connect patients with resources to enhance their care and improve their quality of life.

You Bring:
- Education: A Bachelor’s Degree in Nursing from an accredited institution.
- Experience: 2+ years as an RN with a focus on care coordination, patient education, or disease management.
- Licenses & Certifications: Active State RN License, CPR-BLS, IV certification.
- (ACLS/PALS and Health Coach certification preferred).
- Skills: Proficiency in Microsoft Office and the ability to deliver patient education using a variety of mediums.
- Passion: A genuine interest in educating and empowering patients, coupled with excellent communication and listening skills.
